Delirium Activity Sheets

The Delirium Activity Sheets were designed for use with patients with delirium during their inpatient stay in the hospital. They are intended to support patients in re-orienting to time and place, while providing cognitive puzzles to manage delirium. Multiple versions were designed so that a patient could use by writing directly on the sheet or with the help of their care team and/or family. The sheets have been tested as part of a larger effort focused on delirium management at Brigham & Women’s Hospital and as part of the work by the West Health Accelerator at Mass General Brigham to transform care for older adults.
